Telehealth Disclosures

Effective date: October 22, 2026.

This page explains how CIRx coordinates care and what to know before using telehealth. It supplements our Terms and Privacy Policy.

CIRx and your care team

ConsiderItRx, LLC operates ConsiderItRx.com and coordinates access to care and fulfillment services. Medical evaluations, diagnoses, prescribing and treatment decisions are made by independent licensed clinicians and their medical groups. Licensed pharmacies dispense prescription medication. Laboratory and collection services are provided by the organizations identified for the particular service.

Wizlo-powered technology may support intake, checkout and patient communications, including services using a CIRx-branded web address. The web address does not determine which organization is responsible for clinical care or professional records.

Clinical eligibility and patient location

Service availability depends on the program, your medical needs and your physical location. Your treating clinician must be authorized to provide the relevant care where you are located. Provide accurate health information and location information, and tell your care team when either changes.

Selecting or paying for a program does not guarantee a prescription, a particular medication, dose, formulation or result. A clinician may request further information, testing, a live visit or an in-person evaluation, or may determine that treatment is not appropriate.

Your clinician relies on the information you provide and other information available for your care. Give truthful, accurate, complete and current information, including the health history, medications, allergies and other details requested, and report relevant changes. Inaccurate, incomplete or outdated information can affect treatment decisions and increase the risk of harm. A clinician may need to delay, change, decline or discontinue treatment based on safety concerns, incomplete information or applicable clinical and legal requirements, subject to professional obligations.

Your clinician may require identity verification, additional records, laboratory testing, monitoring, follow-up, a live visit or an in-person evaluation before starting or continuing treatment. Complete the requirements your clinician identifies and follow the directions provided for medication use, testing, monitoring, storage and follow-up.

Telehealth has limits

Remote care may not provide all the information available during an in-person examination. Technology or communication problems can interrupt a visit. Follow your clinician’s instructions if an in-person assessment, testing, referral or different form of care is recommended.

Website content provides general information. It does not replace an individual evaluation or establish a clinician-patient relationship by itself.

CIRx programs do not replace the full range of primary care, preventive care or in-person services you may need. Continue appropriate care with your usual clinicians and obtain an in-person evaluation when recommended.

Review the clinical provider’s telehealth and treatment consent documents and privacy notice before care. This disclosure page is a general explanation and is not a substitute for those documents or any consent required by applicable law.

Use the secure patient channel provided for your care for medical questions and records. Do not put symptoms, medical history, prescription-label images, identification or laboratory results in public search, ordinary email or a marketing signup. For help finding the appropriate care channel, email hello@consideritrx.com without including medical details.

Medication and testing

Only a licensed clinician can decide whether a prescription is appropriate. Follow the directions supplied by your clinician and dispensing pharmacist.

Compounded medications are not FDA-approved. FDA does not review compounded preparations for safety, effectiveness or quality before marketing. Evidence about an FDA-approved product or a different route of administration does not establish equivalent results for a particular compounded formulation.

An FDA-approved medication may sometimes be prescribed off-label, meaning for a use, dose, patient population or route not included in its FDA-approved labeling. FDA approval of the medication does not mean FDA has approved that particular use. Off-label use of an approved medication is different from a compounded preparation, which is not FDA-approved. Ask your clinician about the proposed treatment, its risks, benefits and alternatives.

Laboratory tests have defined scopes and limitations. Results require appropriate interpretation and do not diagnose or exclude every condition. Medication storage instructions and beyond-use dates depend on the actual dispensed product.

Treatment risks and follow-up

Treatment can cause side effects, allergic or other adverse reactions, and complications, and may provide limited or no improvement. The risks, benefits, alternatives and monitoring requirements depend on the specific treatment and your circumstances. Review them with your clinician and ask questions before proceeding.

Contact your treating clinician promptly about new or worsening symptoms or suspected adverse reactions. Ask the dispensing pharmacist about medication identity, condition, storage or temperature concerns. For a medical emergency, call 911 or local emergency services. Do not wait for a portal, email, form, text or chat response.

Billing and cancellation

Treatment subscriptions automatically renew according to the terms disclosed at enrollment. You may cancel future renewals anytime. Review the Terms of Use and Service for prepaid plans, current-order cancellations, returns and refunds. Cancelling billing does not replace medical advice or clinical follow-up.

Emergencies

CIRx website support and administrative email are not emergency services. If you believe you have a medical emergency, call 911 or local emergency services. Do not wait for an email, form, text or chat response.
